Pre-Service Checklist

1. Do I have any COVID-related symptoms? (fever, chills, coughing, chest congestion, shortness of breath or difficulty breathing, fatigue, muscle or body aches, headache, new loss of taste or smell, sore throat, nausea or vomiting, diarrhea, persistent pain or pressure in the chest, new confusion, inability to wake or stay awake, pale gray or blue- colored skin, lips, or nail beds)
2. Have I tested positive for COVID in the past 10 days?
3. Have I been in close contact with or cared for someone who has tested positive in the last 14 days?
